What happened
On January 22, 2018, an experimental Vans Aircraft Inc RV-12, registration N262WS, crashed into a forested area near Bonita Springs, Florida. The flight originated from Page Field (FMY) in Fort Myers, Florida, and was destined for Everglades Airpark (X01).
During the flight, the pilot failed to respond to multiple radio calls from air traffic controllers and used improper radio terminology. While flying under visual meteorological conditions, the pilot climbed to 2,900 feet before being instructed to return to 2,500 feet. At approximately 12:13:49, a controller notified the pilot of nearby traffic. The pilot responded that he was looking for the traffic, and six seconds later, he issued a "mayday" call.
Data from the aircraft's Dynon FlightDEK D180 display showed that the airplane had been operating within the airspeed caution band for much of the flight. Shortly after the pilot reported looking for traffic, the aircraft's pitch changed from 5.25 degrees nose-low to 44.7 and the right roll increased to 37.5 degrees. The airspeed increased from 104 knots to 169.25 knots, which exceeded the aircraft's maximum never-exceed speed of 136 knots. The pilot was fatally injured in the crash.
The investigation
Investigators examined the wreckage and found the debris field measured approximately 715 feet long. The left wing spar exhibited fracture surfaces consistent with overload, and the wing failed in an upward direction, striking the cockpit canopy. There was no evidence of corrosion or fatigue on the fracture surfaces. Flight control continuity was confirmed, and the autopilot pitch servo was found intact and functional.
The engine and propeller were examined, and while the engine could not be rotated manually due to impact damage, no mechanical anomalies or malfunctions were identified. The fuel system was found to be clear of obstructions, though a small piece of pine straw was found inside the fuel pump bowl.
Medical examinations of the pilot revealed significant coronary artery disease, with 50% to 75% narrowing of the left main and left anterior descending coronary arteries. Toxicology testing identified several psychoactive medications in the pilot's system, including antidepressants. Medical records also indicated the pilot had been treated for hypertension, high cholesterol, obstructive sleep apnea, depression, and PTSD.
