Incident Overview
An incident involving a Boeing MD-11F aircraft, operated by United Parcel Service (UPS), occurred during a flight over Poland. The aircraft is powered by three Pratt & Whitney PW 4460 turbofan engines. During cruise, the No. 2 engine, located on the right side, experienced fluctuations and subsequently flamed out. The flight continued to its destination, where after landing, fire and smoke were observed emanating from the No. 2 engine.
Response
The airport fire brigade was dispatched to the scene and applied fire suppression to the affected engine. Following the suppression, the aircraft was towed from the runway for further examination. No injuries were reported among passengers or crew.
Investigation
The government of Poland has notified the National Transportation Safety Board (NTSB) of the incident. The NTSB has appointed a U.S. Accredited Representative to assist the Polish authorities under the provisions of ICAO Annex 13, as the United States is the state of manufacture of the engine. All investigative information will be released by the government of Poland. No probable cause has been determined at this time.
