Robinson R44 II Helicopter Destroyed in Solo Flight Accident at EPPL

Casualties unknown • EPPL, PL

A student pilot's solo training flight ended in a helicopter rollover and subsequent fire at Płock Aerodrome after an improper takeoff maneuver.

What happened

On 27 March 2022, at the Płoded Aerodrome (EPPL), an instructor was conducting training flights for the Private Pilot License (Helicopter) program. Following a successful pre-flight inspection and an initial supervised circuit flight, the instructor exited the R44 II helicopter, registration SP-INW, and authorized the student-pilot to perform a solo flight along the aerodrome traffic circuit.

During the attempt to take off and establish a hover, the helicopter began to roll toward the right while simultaneously turning left around its vertical axis. The right skid of the rear section made contact with the ground, followed immediately by the main rotor blades striking the surface. This impact caused the helicopter to dynamically overturn onto its right side. The student-pilot was ejected from the aircraft but managed to walk away to a safe distance without sustaining any injuries. Shortly after the impact, a fire broke out, which resulted in the total destruction of the aircraft. Emergency services arrived at the scene and successfully extinguished the flames.

The investigation

The PKBWL investigation examined the sequence of events leading to the rollover and the subsequent fire. The investigation confirmed that the student-pilot was not under the influence of alcohol and that the aircraft possessed all required documentation and insurance. Medical personnel from the HEMS team examined the student at the aerodrome and found no injuries.

Findings

The investigation established that the primary cause of the accident was the delayed reaction of the student to the right-side roll, combined with an abrupt upward movement of the collective lever.

Safety action

Following the accident, the aircraft operator implemented preventive training measures for their personnel, focusing on errors occurring during helicopter take-off and the necessary corrective actions to prevent similar occurrences.

Probable cause

The accident was caused by the student pilot's delayed response to a rightward roll and an improper, abrupt application of upward collective pitch during takeoff.
