Accident Overview

On October 23, 2022, at approximately 2310H local time (1510 UTC), an Airbus A330-322 aircraft with registration HL7525, operated by Korean Air Lines Co., Ltd., experienced a runway overrun during the landing roll on Runway 22 at Mactan-Cebu International Airport (RPVM), Lapu-Lapu City, Cebu, Philippines. The flight was a scheduled commercial operation originating from Incheon International Airport, Korea. Instrument Meteorological Conditions (IMC) prevailed at the time of the accident.

Injuries

According to the final report, one flight crew member, four cabin crew members, and fifteen passengers sustained minor injuries.

Sequence of Events

The aircraft made multiple approaches to Runway 22. During the second approach, the aircraft experienced ground contact before the runway threshold. The right-hand main landing gear (MLG) struck a 15-centimeter step at the cemented edge of Runway 22, resulting in multiple damages and faults to the landing gear. These damages led to the loss of most deceleration means, specifically aircraft braking failure, which caused the runway overrun.

Cause Factors

The Aircraft Accident Investigation and Inquiry Board (AAIIB) determined that the cause factors were:

  • An increase in vertical speed indicator (VSI) attributed to forward pitch control from the captain, resulting in ground contact before the runway.
  • An increase in vertical wind factor during the aircraft's descent on the second approach.
  • The right-hand MLG hitting a 15-centimeter step of the cemented edge of Runway 22, causing multiple damages to the landing gear and subsequent loss of braking capability.

Investigation

The investigation was conducted by the AAIIB of the Civil Aviation Authority of the Philippines (CAAP) in accordance with ICAO Annex 13. Accredited representatives from France (BEA), Republic of Korea (ARAIB), United Kingdom (AAIB), Switzerland (STSB), and the United States (NTSB) participated. The final report was released on March 7, 2025.

The cause factors of this accident were attributed to the increase of VSI attributed to the forward pitch control from the captain that resulted in the ground contact before the runway, the increase in vertical wind factor during the aircraft’s descent on the second approach, and the right-hand MLG hitting a 15-centimeter step of the cemented edge of RWY 22, resulting in multiple damage/faults to the said landing gear and consequently the loss of most of the deceleration means, specifically aircraft braking failure resulting in runway overrun.
