Fatal crop-spraying accident near Olkhovatka

2 fatalities • Breusovka, Ukraine • Takeoff (climb)

An aircraft crash during agricultural operations in the Poltava region resulted in two fatalities and one serious injury following engine failure.

What happened

During a series of crop-spraying missions based out of Olkhovatka, an An-2 aircraft performed an unapproved flight segment from Breusovka to Olkhovatka. At the time of this maneuver, the aircraft was carrying two unauthorized passengers in addition to the pilot.

Shortly after departing for the mission, the aircraft experienced significant mechanical issues. The engine began malfunctioning, leading to a loss of power during the initial climb. In an attempt to manage the emergency, the pilot directed the plane toward an open field for an unplanned landing. However, the impact with the ground was severe, causing the airframe to disintegrate upon contact.

The accident resulted in two fatalities and one serious injury. The deceased included the pilot and one of the passengers, while the remaining individual sustained critical injuries.

Findings

Investigations into the crash identified that the engine failure was triggered by water contamination within the fuel supply. Additionally, post-accident analysis revealed that the pilot was operating the aircraft while heavily intoxicated, with a blood alcohol concentration measured at 3.8 ‰.

Probable cause

The accident was caused by engine failure resulting from water in the fuel, compounded by the pilot's extreme level of intoxication.
