A four-engine aircraft veered off the runway and suffered landing gear collapse following an engine failure during its takeoff roll.

What happened

During the acceleration phase of a takeoff roll, a four-engine aircraft experienced a sudden deviation to the left. In an attempt to maintain the centerline, the captain utilized right rudder and adjusted the throttles, specifically advancing the power on the remaining engines while managing the lead of the first engine. During this corrective maneuver, the number one engine suffered a failure.

Despite the crew's efforts to compensate for the loss of power, the aircraft became uncontrollable and drifted toward the right side of the runway. As the plane exited the paved surface, the right main landing gear collapsed. The incident resulted in zero fatalities and zero injuries among the seven crew members on board. The aircraft sustained damage that rendered it a total loss.

  • The primary factor in the loss of directional control was the failure of the number one engine during the takeoff sequence.

Probable cause
