Accident Details
Approximately ten minutes after takeoff, the pilot stated he was returning to land due to a problem. During final approach, the aircraft rolled inverted and descended nose-first into a residence. Both occupants were killed. The landing gear was down and flaps were extended about 20 degrees. Witnesses observed the aircraft descend below traffic pattern altitude. At about 200 feet above ground level, a loud engine noise was heard; the slow-flying aircraft then pitched up, rolled right, and descended vertically about half a mile from the airport. The pilot did not declare an emergency or specify the nature of the problem.
Investigation Findings
Engine examination revealed a failed supercharger intermediate drive shaft gear on the right engine, resulting in loss of supercharger function. The aircraft was approximately 300 pounds over its maximum gross weight. The left throttle was found in the full forward (high power) position. The right propeller control was at full decrease rpm (high pitch), and the right engine magneto switches were off.
The pilot's last recorded flight in a Beech 65 was in February 1988, and there was no record of a check-out or flight training in the aircraft.
Probable Cause
The official probable cause was determined as follows:
- Occurrence #1: Partial loss of engine power due to mechanical failure/malfunction, caused by a worn accessory drive gear and total failure of the turbocharger system.
- Occurrence #2: Loss of control in flight during approach, attributed to inadequate in-flight planning/decision by the pilot, failure to maintain airspeed (Vmc), inadequate training in emergency procedures, lack of recent experience in the aircraft type, and exceeding aircraft weight and balance limits by company/operator management.
- Occurrence #3: In-flight collision with a residence.
