What happened
During the takeoff sequence at the airport, observers noted that the aircraft type was positioned at the end of runway 2ឡើងR with its tail extending into the overrun area. The aircraft underwent an unusually long takeoff roll before achieving lift-off. Immediately following rotation, the plane pitched upward vertically and reached an altitude of approximately 100 feet. The aircraft then rolled into an inverted position to the left, entered a dive on a heading of 035°, and struck the ground, subsequently bursting into flames. There were two fatalities among the occupants.
Findings
Investigations determined that the aircraft was operating at a weight exceeding its maximum gross limit by 955 pounds. Additionally, the center of gravity was located 14.5 inches behind the most rearward allowable limit. To hide these discrepancies, the weight and balance documentation had been falsified to show the plane was within legal operational limits prior to departure.
Further examination of FAA records showed that Air Cargo America had a history of regulatory violations. A pilot from the company reported that management exerted pressure on crews to fly in violation of federal air regulations. The investigation identified several contributing factors, including inaccurate weight and balance documentation, poor pilot judgment, and inadequate oversight by both the operator and the FAA.
