What happened
During a departure from Lansing, Michigan, the pilot reported experiencing mechanical issues with the engines. Shortly after the report of engine problems, the aircraft suffered a complete loss of power to both engines. Investigations into the aircraft's recent maintenance and fueling history revealed that the plane had been serviced with 25 gallons of jet fuel in each wing tank.
Findings
An investigation by the FAA included interviews with both the individual responsible for fueling the aircraft and the safety director of the fueling service provider. The fueling personnel confirmed that a JET-A fuel truck was used for the operation. However, the safety director disclosed that the specific truck used during the incident was equipped with a small nozzle on one of its hoses, rather than the standard wide nozzle typically found on jet refueling equipment. This small nozzle was intended for refueling airport tugs. Following the incident, the safety director stated that these small nozzles were removed from all jet refueling trucks in the fleet to prevent a recurrence of the incorrect nozzle usage.
