A seaplane sank rapidly after striking waves during a takeoff attempt off Fort Walton Beach, resulting in three fatalities and four rescues.

What happened

The incident occurred while the crew was conducting training maneuvers involving takeoffs and landings on rough seas near Fort Walton Beach. During the initial phase of a takeoff, the aircraft struck significant wave action, causing it to plunge into the ocean. The crash site was located approximately 15 miles offshore.

Upon impact, the seaplane began to sink quickly. A critical factor in the rapid submersion was that only one watertight door had been secured prior to the accident. This allowed a large volume of water to flood the interior of the fuselage almost immediately. Emergency responders managed to rescue four crew members from the water, but three others perished in the incident. The wreckage was never recovered from the seabed.

Findings

Investigations indicated that the pilot in command was undergoing a transition at the time of the event. It was determined that the individual was practicing maneuvers in violation of applicable regulations.

Probable cause

The pilot in command was practicing against regulations during a transition phase.
