What happened
During a departure from runway 25 at DuBois-Jefferson County Airport, a Douglas DC-3 experienced a loss of control. The aircraft drifted left off the runway surface, descended down an embankment, and eventually stopped within a nearby wooded area. The impact resulted in the total destruction of the airframe. There were 11 injuries reported among the occupants, though no fatalities occurred.
Findings
Investigations into the accident identified several contributing factors related to pilot performance and mechanical configuration. At the time of takeoff, the aircraft was operating with a 7-knot crosswind and an unlocked tailwheel.
Key findings regarding the crew include:
- The pilot in command lacked the necessary experience and qualifications to operate the Douglas DC-3.
- The pilot occupying the right seat failed to take control of the aircraft in a timely manner during the excursion.
- There was a failure in oversight, as the pilot in command allowed an unqualified individual to perform the takeoff sequence.
- Operational procedures were bypassed when the Regional Director occupied the left seat, which deviated from established rules intended to ensure only qualified personnel occupy specific duty stations.
