Incident Overview

On 24 June 2023, a Cathay Pacific Airways Boeing 777-300ER, registration B-KPQ, experienced an explosive tire deflation prior to an evacuation at Hong Kong International Airport (VHHH). The Air Accident Investigation Authority (AAIA) of Hong Kong is conducting the investigation. An interim statement was released on 24 June 2026, as required by Annex 13 to the Convention on International Civil Aviation when a final report cannot be published within twelve months.

Evacuation and Personnel

According to the AAIA, the explosive tire deflation occurred before the evacuation. Two maintenance personnel who initially attended the aircraft narrowly escaped serious injury.

Safety Recommendations

During the investigation, the AAIA identified issues in communication and coordination between the operator, Cathay Pacific Airways, and its maintenance and ground service providers. These issues were considered a safety risk requiring timely mitigations. Consequently, a Safety Recommendation was made to Cathay Pacific and accepted by the operator. The recommendation calls for a systemic review of ground handling procedures, specifically focusing on the risks associated with a high-speed rejected takeoff (RTO). Additionally, the operator should establish effective mechanisms for communication and coordination with ground service providers, including both in-house and contracted personnel, and ensure their familiarity with safety precautions outlined in relevant manuals.

Technical and Human Factors Investigation

The investigation has undertaken comprehensive testing to ascertain the reason for the explosive tire deflation. In addition to technical aspects, the investigation examined various human factors associated with the accident. This included an assessment of the crew decision-making process, interaction among crew members, training protocols, and the emergency evacuation procedures followed by the personnel involved.
