What happened
On 30 August 2022, a Cessna 208B EX, registration PK-SNW, was performing a scheduled cargo and passenger flight from Timika to Sinak, Papua. The aircraft, operated by PT Smart Cakrawala Aviation, was on its seventh flight of the day, carrying two pilots, two passengers, and over 1,000 kg of cargo.
During the final approach to Runway 35, the aircraft's Terrain Avoidance Warning System (TAWS) issued multiple "Sink Rate" and "Pull Up" alerts. The aircraft touched down on the runway and bounced, subsequently touching down a second time. During the landing roll, the pilot applied reverse thrust and brakes, but the aircraft could not be stopped within the remaining runway length. The aircraft eventually struck an embankment beyond the end of the runway. All occupants evacuated the aircraft safely, though the aircraft sustained damage.
The investigation
The investigation examined the flight data from the Garmin G100 and cockpit voice recordings. Investigators analyzed the aircraft's approach profile, including airspeed and descent rates, and compared them against the required stabilized approach criteria. The study also reviewed the aircraft's braking performance and the environmental conditions at Sinak Airport, specifically the high elevation and temperature. Additionally, the investigation looked into the crew's decision-making processes and the presence of potential distractions in the cockpit, such as the use of music during critical flight phases.
