What happened
On 24 May 2022, a Cessna 208B, registration PK-BVE, operated by PT. ASI Pudjiastuti Aviation (Susi Air), was performing an unscheduled passenger flight from Malinau to Yuvai Semaring. The flight carried 12 passengers. After landing on Runway 22, the crew performed a 180-degree turn to vacate the runway via Taxiway C.
As the aircraft taxied toward the apron, the pilot in command, who had not visited the airport in eight months, discussed the parking location with the second in command. Seeking to facilitate easy passenger disembarkation and leave space for subsequent arrivals, the pilot decided to park the aircraft near the terminal building, close to the apron edge. During this maneuver, the left-wing leading edge of the aircraft struck an apron floodlighting pole. There were no injuries to the passengers or crew, though the aircraft sustained damage.
The investigation
The KNKT investigation examined the aircraft's movement, the airport's infrastructure, and the regulatory oversight of the facility. Investigators found that the aircraft was operating in clear weather with 5 km visibility. The investigation focused on the lack of visual guidance on the apron, the visibility of the obstacle, and the accuracy of aeronautical information provided to pilots.
Findings
- The floodlighting pole was difficult to see due to its inconspicuous coloring and a lack of prior communication regarding its presence.
- The pilot's situational awareness was compromised by the absence of taxiway centerline and aircraft parking stand markings on the apron.
- The aircraft's maneuver brought it too close to the obstacle because the crew lacked clear guidance for navigating the apron area.
- The airport's Aeronautical Information Publication (AIP) contained an incorrect coordinate for the Aerodrome Reference Point.
- The floodlighting pole, located in the aerodrome's Area 3, was not listed as an obstacle in the official aeronautical documentation.
Safety action
Following the incident, Susi Air issued safety notices to its crew regarding sterile cockpit procedures, approach briefings, and the importance of airport familiarity. The Yuvai Semaring Airport operator has since painted taxiway centerline and apron markings to comply with safety standards. The KNKT also issued recommendations to the Directorate General of Civil Aviation to improve oversight of airport non-compliance and to ensure that all obstacles in Area 3 are properly identified and published.
