What happened
During the initial climb phase after departing from Vilyuisk Airport, the aircraft's left engine ceased functioning. In response to the malfunction, the flight crew performed procedures to shut down the engine and feather the propeller. The decision was made to return to the departure airfield to perform a landing.
While the aircraft was on its final approach to the airport, it entered a stall and impacted a field. The accident resulted in two fatalities and left 14 other occupants injured. The impact led to the total destruction of the aircraft.
Findings
A technical issue involving the regulator caused the failure of the left engine shortly after takeoff. Investigations revealed that the crew's response was characterized by a lack of coordination and a failure to execute necessary procedures correctly. This resulted in the aircraft being operated in an improper configuration with insufficient airspeed during the approach, which ultimately led to the aerodynamic stall. Additionally, it was noted that the pilot and the instructor in the right seat lacked the necessary proficiency to manage the flight conditions while operating with one engine inoperative.
