What happened
A cargo flight traveling from Helena to Great Falls ended in a fatal accident during the final stages of the approach. The aircraft, operated by a single pilot, was performing a nighttime delivery mission when it struck the ground several miles before reaching the runway threshold. The impact resulted in the total destruction of the aircraft and the death of the pilot, who was the only person on board.
At the time of the crash, meteorological conditions were significantly below established flight minimums. The flight crew encountered difficult weather while attempting to land at the destination.
Findings
Investigations into the accident identified several critical factors that led to a controlled collision with the ground during the final approach. Key contributing elements included:
- Presence of fog and visibility restricted to one-half mile or less
- Failure to adhere to established approach certification specifications
- Improper IFR operation during the descent
- An excessive rate of descent
- Maintaining an excessive approach speed
- Descending below required minimum altitudes
