An aircraft veered off the runway and broke in two after an erroneous engine failure warning triggered an emergency braking maneuver.

What happened

During the takeoff roll, approximately 38 seconds after the crew began procedures, the aircraft reached a speed of 230 km/h and traveled 1,450 meters when an alarm signaled a failure in the right engine. In response to this warning, the captain assumed control and began emergency braking. Although thrust reversers were engaged seven seconds later to assist in deceleration, the aircraft continued toward the end of the runway.

To prevent a collision with the instrument landing system (ILS) and approach lighting equipment, the pilot steered the plane to the left of the runway. Upon exiting the paved surface, the aircraft encountered soft terrain, which caused the landing gear to detach. The plane subsequently struck a drainage ditch and split into two sections. The wreckage came to a stop roughly 530 meters past the runway threshold and 47 meters left of the centerline.

Of the occupants on board, one fatality was recorded due to a passenger suffering a heart attack. Additionally, seven passengers sustained injuries during the excursion, while 57 others were able to exit the aircraft without harm.

Findings

Investigations revealed that the right engine had actually been functioning correctly throughout the takeoff. The primary factor in the accident was a malfunction of the cockpit warning system that triggered a false engine failure alert.
