  • 2014-05 A B727 Second Officer alerted the Captain about the Pressurization AUTO Controller failure on final approach; but the Captain dismissed him and he later learned the failure was not entered in the Maintenance Log.
  • 2011-06 Weather; political border issues; US Customs and a company Duty Officer participating from a house party conspired to keep a B727-200 flight crew on duty for 23 hours.
  • 2010-06 A B727-200 Captain reported that the Flight Engineer's freight loading schedule was not completed as requested but he failed to change the Weight and Balance to reflect the as loaded condition. On takeoff; the Captain had to roll in nose down trim to prevent a tail strike. The Captain believed the Flight Engineer's advanced age was factor.
