- 2025-10 Air carrier technicians reported that while replacing an emergency power assist system battery on a B777; the door opened and the escape slide released but did not fully deploy.
- 2025-08 Air carrier technicians were performing a passenger entry door operational test of the B777 emergency power assist battery. The squib fired and escape slide deployed.
- 2025-07 Flight Attendants reported an escape slide deployment near miss when a main entry door was opened prior to doors being disarmed. Flight attendants were waiting for the seat belt sign to be extinguished prior to disarming doors.
