    A CESSNA C525 LANDED ON RUNWAY 33 AND WAS INSTRUCTED BY LOCAL (LC) TO TURN LEFT ON TAXIWAY ECHO. THE C525 PILOT CONTACTED GROUND (GC) AND WAS INSTRUCTED TO TURN LEFT ON DELTA AND LEFT ON FOXTROT, CROSS RUNWAY 3 AND HOLD SHORT OF RUNWAY 15. (ECHO ON THE EAST SIDE OF RUNWAY 33 IS CLOSED FOR CONSTRUCTION AND RUNWAY 3 HAS BEEN CLOSED FOR APPROXIMATELY 8 MONTHS FOR CONSTRUCTION). THE C525 PILOT READ BACK THE CLEARANCE. AS THE C525 APPROACHED THE LEFT TURN ON FOXTROT THE PILOT QUESTIONED GC ON HIS CLEARANCE AND WAS TOLD TURN LEFT ON FOXTROT AND HOLD SHORT OF RUNWAY 33. THE CLEARANCE AND HOLD SHORT WAS READ BACK BUT NO CALL SIGN WAS GIVEN DURING THE READ BACK. THE C525 DID NOT HOLD SHORT, ENTERED RUNWAY 33 AND CONFLICTED WITH A CANADAIR CRJ2 ON DEPARTURE ROLL RUNWAY 33 FULL LENGTH (APPROXIMATELY 1,800 FEET FROM FOXTROT). IT APPEARS THAT CRJ2 PILOT AND LC SAW THE CONFLICT AT THE SAME TIME AS THE PILOT ABORTED AND AS LC INSTRUCTED THE CRJ2 TO ABORT. THE C525 PILOT SAW THE CRJ2 ON DEPARTURE ROLL AND ACCELERATED ACROSS AND OFF THE RUNWAY. THE CRJ2 SWERVED TO PASS BEHIND THE C525. CLOSEST HORIZONTAL PROXIMITY REPORTED WAS ESTIMATED AT 20 FEET.

    LOCAL CONTROL 1 (LC1) WAS WORKING RUNWAY 25R AND ASKED LOCAL CONTROL 2 (LC2) IF THEY COULD OFF-LOAD SOME OF THE NORTH TRAFFIC TO RUNWAY 25L. THE REQUEST WAS APPROVED. LC 2, RESPONSIBLE FOR RUNWAY 25L, CLEARED A QUICKIE Q200 (HXA EXPERIMENTAL) TO LAND ON RUNWAY 25R BY MISTAKE. THE Q200 PILOT READ BACK CLEARED TO LAND WITHOUT STATING THE RUNWAY ASSIGNMENT. LATER LC2 ASKED THE Q200 PILOT TO VERIFY LANDING ON RUNWAY 25L BUT THE Q200 PILOT READ BACK RUNWAY 25R. LC2 MISSED THE INCORRECT READ BACK. LC1 INSTRUCTED A CESSNA C152 TO TAXI INTO POSITION AND HOLD (TIPH) ON RUNWAY 25R AT APPROACH END FOR A FULL LENGTH DEPARTURE. THE Q200 LANDED OVER THE C152 HOLDING ON THE RUNWAY BY LESS THAN 100 FEET VERTICAL (ESTIMATED 50 FEET) AND TOUCHED DOWN APPROXIMATELY 200 FEET DOWN THE RUNWAY. THE C152 STATED ONE FREQUENCY THAT IT LOOKED LIKE AN AIRCRAFT LANDED OVER THEM. THE Q200 DID NOT SEE THE C152 ON THE RUNWAY AT APPROACH END.

    A BEECH BE9L (C90) WAS TAXIED TO SOUTHEAST RAMP VIA TAXIWAY BRAVO, ROMEO, TO HOLD SHORT OF RUNWAY 9C. THE BE9L WAS SUBSEQUENTLY INSTRUCTED TO TURN RIGHT ON KILO AND TAXI VIA KILO, CHARLIE, ROMEO AND TO HOLD SHORT OF RUNWAY 9R. ALL READ BACK WERE CORRECT. APPROXIMATELY ONE MINUTE LATER THE BE9L TURNED LEFT FROM KILO INSTEAD OF CONTINUING TO CHARLIE, ENTERED THE OVERRUN FOR RUNWAY 9C AND TAXED TOWARDS THE APPROACH END THUS CONFLICTING WITH A PIPER P32R ON SHORT FINAL SAME RUNWAY. THE P32R WAS LESS THAN ONE QUARTER (.25) MILE FINAL WHEN INSTRUCTED TO GO AROUND AND ADVISED OF THE BE9L IN THE OVERRUN. THE BE9L WAS APPROXIMATELY HALF WAY BETWEEN KILO AND THE APPROACH END OF RUNWAY 9C WHEN INSTRUCTED TO HOLD POSITION AND PILOT COMPLIED. IT APPEARED THE P32R FLEW OVER THE BE9L ON THE GO AROUND BY AN ESTIMATED 50 FEET VERTICAL.

    A FLIGHT OF TWO, A LUSCOMBE L8 AND A AERONCA AR11 WERE HOLDING SHORT OF RUNWAY 17L AT TAXIWAY A1 FOR DEPARTURE. THE L8 PILOT REPORTED READY AND GROUND CONTROL TOLD THE PILOT HE WAS NUMBER ONE FOR DEPARTURE AND TO MONITOR TOWER FREQUENCY. THE L8 THEN ENTERED RUNWAY 17L AT A1 WITHOUT CLEARANCE AND CONFLICTED WITH A CESSNA C172 LANDING SAME RUNWAY. THE C172 FLEW OVER THE NOSE OF THE L8 AS IT WAS ENTERING THE RUNWAY BY AN ESTIMATED 50 FEET VERTICAL AND LANDED NORMALLY APPROXIMATELY 1,000 FEET DOWN THE RUNWAY. THE AR11 DID NOT ENTER THE RUNWAY.

    AN F15 LANDED RUNWAY 4R AND WAS INSTRUCTED TO TURN LEFT ON TAXIWAY KILO, CROSS RUNWAY 8L. THE F15 PILOT READ BACK THE INSTRUCTIONS. APPROXIMATELY 2 MINUTES AFTER THE F15 HAD LANDED A BOEING B767 LANDED ON RUNWAY 4R. THE B767 PILOT REPORTED AN AIRCRAFT ON THE RUNWAY AHEAD OF THEM. LOCAL CONTROL DID NOT ENSURE THE RUNWAY WAS CLEAR AND CLEARED THE B767 TO LAND. BROADCAST WAS MADE IN THE BLIND AND IT WAS DETERMINED TO BE THE F15 THAT WAS INSTRUCTED TO EXIT AT KILO AND TO CROSS RUNWAY 8L. THE B767 PILOT STATED THE FIRST THEY SAW OF THE F15 WAS THE TAILPIPES AND THEY BRAKED HARD AT ABOUT 70-80 KNOTS AND WHEN THEY STOPPED THEY WERE LOOKING DOWN ON THE F15. B767 PILOT STATED THE F15 WAS AT THE HOLDING POSITION MARKING ON RUNWAY 4R SHORT OF RUNWAY 8L AND ORIENTED DOWN RUNWAY 4R. THE LAHSO LIGHTS ON 4R SHORT OF 8L ADDED TO THE DIFFICULTY OF SEEING THE F15. CLOSEST HORIZONTAL PROXIMITY WAS LESS THAN 200 FEET. F15 PILOT REPORTED CANOPY WAS FOGGED.

    OSH AIRVENTURE FLY-IN PROCEDURES WERE IN EFFECT. A SR22 WAS LANDING RUNWAY 27 FULL LENGTH. THE CONTROLLER WORKING ITINERANT MOBILE (IM) FLAGGING AT TAXIWAY ALPHA AND RUNWAY 27 DID NOT SEE THE SR22 LANDING FURTHER UP FIELD THAN PREVIOUS ARRIVALS HAD CLOSER TO THE RUNWAY NUMBERS AND FLAGGED A VEHICLE (EL CAMINO) TO CROSS 27 AT ALPHA. THE VEHICLE DRIVER SAW THE SR22 AND STOPPED ON THE RUNWAY. THE SR22 WING TIP PAST IN FRONT OF THE VEHICLE. CLOSEST HORIZONTAL PROXIMITY REPORTED WAS 5 FEET.

    RUNWAY 35 WAS CLOSED AND ADVERTISED ON THE ATIS. A CESSNA C210 LANDED ON CLOSED RUNWAY 35 WITHOUT CLEARANCE OR CONTACT WITH FTG ATCT AND CONFLICTED WITH A VEHICLE ON THE RUNWAY. AIRPORT OPERATIONS WAS WORKING ON RUNWAY LIGHTING AND VEHICLE WAS PARKED ON EAST EDGE OF RUNWAY 35 (INSIDE EDGE LINES) NEAR INTESECTION D3 FACING NORTH. THE OPS PERSON WAS OUT OF THE TRUCK AND OBSERVED THE C210 APPROXIMATELY 50 FEET AGL COMING IN TO LAND. THE C210 LANDED APPROXIMATELY 1,000 FEET FROM THE VEHICLE AND OPERATOR THEN THE AIRCRAFT MANUEVERED WEST TO AVOID THE TRUCK AND CAME APPROXIMATELY 50-75 FEET OF THE VEHICLE. THE OPS INDIVIDUAL ESTIMATED THE C210 PASSED HIS POSITION BETWEEN 55-60 MPH (FREEWAY SPEED) THEN EXITED AT D6. PILOT STATED ELECTRICAL AND RADIO PROBLEMS, WAS AWARE RUNWAY 35 WAS CLOSED BUT FELT IT SAFER TO USE 35.

RUNWAY 11R HAD AN ARRIVAL THAT LANDED AND THE NEXT DEPARTURE, A PIPER PA28A, WAS TAXIED INTO POSITION AND HOLD (TIPH) RUNWAY 11R AT APPROACH END. LOCAL CONTROL (LC) NOTICED ON SHORT FINAL FOR RUNWAY 11R AN AIRCRAFT THAT TURNED OUT TO BE THE PA28A FOR 11L AND ISSUED GO AROUND TO THE PILOT BUT TO NO AVAIL. THE LANDING PA28A OVER FLEW THE PA28A HOLDING IN POSITION ON RUNWAY 11R AND LANDED NEAR TAXIWAY C4. CLOSEST PROXIMITY REPORTED WAS 50 FEET VERTICAL. THE DISTANCE FROM APPROACH END 11R TO C4 IS APPROXIMATELY 1,400 FEET.
