On September 12, 1991, a Cessna 401 (registration N13DT) operated by Montague, Harvey L. was involved in an aviation accident near Jackson, TN. Investigators recorded the probable cause as: The pilot had failed to switch from the auxiliary fuel tanks to the main fuel tanks prior to the approach, and exhausted the auxiliary fuel supply. This summary draws on records from the U.S. National Transportation Safety Board (NTSB) historical archive; 1 related events involving the same aircraft type or operator are linked below.
During a visual approach after canceling an IFR flight plan, both engines failed due to fuel exhaustion from auxiliary tanks. The pilot force-landed in a bean field short of the airport; occupants egressed before the fuselage was consumed by fire.
Incident Overview
The pilot had canceled his instrument flight plan and was navigating to the airport using visual references. While on final approach, the left engine ceased operation; immediately afterward, the right engine also lost power. The pilot then recognized that he had neglected to change the fuel selector from the auxiliary tanks to the main tanks before beginning the approach, resulting in complete depletion of the auxiliary fuel. Because the aircraft did not have sufficient altitude to attempt an in-flight restart of the engines, the pilot executed a forced landing in a bean field located short of the airport runway.
Aftermath
After the airplane came to a complete stop, the occupants evacuated the aircraft. Subsequently, a post-crash fire ignited and consumed the entire fuselage.
Probable cause
The pilot had failed to switch from the auxiliary fuel tanks to the main fuel tanks prior to the approach, and exhausted the auxiliary fuel supply.
