Accident Overview
A mountain flying training flight experienced a takeoff accident at an airport with an elevation of 9,927 feet above mean sea level and a density altitude of approximately 12,400 feet. The certified flight instructor (CFI) for the flight observed that the airport's elevation exceeded the maximum values available on the takeoff performance charts.
Takeoff Sequence
During the initial takeoff roll, the pilot rotated the aircraft, but it settled back to the ground on three separate occasions. On the fourth rotation, which occurred near the departure end of the runway, the aircraft lifted off. Shortly after becoming airborne, it descended along the slope of an incline, collided with a power line, and finally came to rest on flat, rocky terrain. The three occupants exited the aircraft with injuries.
Investigation Findings
The subsequent investigation identified several factors. The airport's elevation was above the highest altitude provided in the takeoff data charts. The engine mixture was not leaned according to the procedures specified in the aircraft's flight manual. Additionally, the flaps were not placed in the correct setting for the type of takeoff being conducted. The investigation also established that the flight instructor had not undergone any formal mountain flying training. According to the investigation, the Federal Aviation Administration (FAA) does not offer approved mountain flying training courses, and there is no requirement for specialized training to act as an instructor in mountain flying.
