Incident Overview

Shortly after takeoff, the flight crew encountered a compressor stall in the No. 2 engine, followed by a loss of engine power and an abnormal oil pressure reading. The engine was shut down, and the pilots performed an emergency landing at John F. Kennedy International Airport (JFK). Debris from the incident came to rest in a residential area, where it struck a vehicle.

Engine Examination

Post-incident examination of the engine revealed that turbine parts had punctured through the turbine exhaust case at several locations, but were contained by the fan exit duct. All fourth-stage turbine blades had fractured across the airfoil just above the platform and were retained in the disk. Of the twenty-five fourth-stage turbine vane clusters, twelve were missing, and the remaining clusters exhibited considerable trailing edge damage. All third-stage turbine blades and vanes showed minor secondary damage, with no blade or vane damage observed forward of the third stage.
