A commuter flight departing runway 36 experienced a loss of control after flap retraction, leading to a landing short of the runway end and substantial damage. The pilot had removed ice and snow during preflight but did not recheck before departure.

Accident Sequence

The accident occurred during the departure of a scheduled commuter flight from runway 36. The aircraft was occupied by a certificated airline transport pilot and one pilot-rated passenger. According to the pilot, as the airplane climbed to approximately 100 feet above the runway, he retracted 10 degrees of flaps. When the airplane reached about 200 feet, he retracted the remaining 10 degrees of flaps. Immediately thereafter, the airplane began to descend while rolling to the left.

The pilot stated that he applied full right aileron in an attempt to keep the airplane upright, but the descent continued. He noted that full engine power was already applied. The airplane subsequently landed on the runway, roughly 200 feet from the threshold, and continued off the end of the runway into snow-covered terrain. The accident resulted in substantial damage to the propeller, fuselage, and wings.

Post-Accident Pilot Statements

The pilot reported that during his preflight inspection, he removed an accumulation of ice and snow from the wings and empennage. However, due to scheduling delays, the flight did not depart until about two hours after the initial preflight. The pilot stated that he did not recheck the aircraft for any additional frost accumulation before departure.
