Incident Overview
On 28 October 2013, at approximately 1650 UTC, an ATR-42-500 aircraft, registration PR-TTK, operated by Trip Linhas Aéreas Brasil, experienced an engine failure shortly after takeoff. The flight was a ferry operation from SBIP to SBBH, carrying only the captain and first officer.
Sequence of Events
During the initial climb, the number 2 engine failed at an altitude of about 400 feet above ground level. The crew initiated a return to SBIP. While on the downwind leg, the number 1 engine exhibited torque and parameter fluctuations. The crew then shut down the number 1 engine. The aircraft subsequently landed on the runway, during which a tire blowout occurred.
Investigation
The incident is under investigation by the Aeronautical Accident Prevention and Investigation Center of Brazil (CENIPA). The National Transportation Safety Board (NTSB) has appointed an Accredited Representative to assist, as the United States is the State of Manufacture of the engines, in accordance with ICAO Annex 13. All inquiries should be directed to CENIPA.
