What happened
On October 7, 2024, at 17:30 EDT, a Bell 206L-3 helicopter, registration N230AE, crashed in Owenton, Kentucky, while performing an air medical flight. The flight, operated by Air Evac Life Team, was requested at 17:14 to a landing zone (LZ) adjacent to a local high school. During the approach, the pilot communicated that the crew would circle the LZ until necessary fire units arrived to prepare the site.
At approximately 17:29, the pilot reported that the high school was in sight and intended to perform reconnaissance before landing. Witnesses observed the helicopter approaching from the north at a low altitude. During this approach, the aircraft struck a guy wire belonging to a 601-foot television signal transmission tower located on the high school campus. Witnesses reported seeing the main rotor system separate from the helicopter following the impact. The helicopter and the rotor system subsequently descended to the ground separately. The crash resulted in 3 fatal injuries, claiming the lives of the pilot, a flight nurse, and a flight medic.
The investigation
Investigators examined the wreckage at the accident site and accounted for all major components. The fuselage came to rest on its left side approximately 6/70 feet from the tower base and was destroyed by impact and fire. The tailrotor and tailrotor gearbox separated from the airframe, and one blade was separated from its grip.
Mechanical examination of the flight controls and hydraulic servos revealed fractures and bending consistent with overload. The main rotor mast was found fractured at its base near the top of the transmission case, also showing features of overload separation. The engine, a Rolls-Royce M250-C30P, remained mounted but sustained fractured mounts and damage to the KAflex driveshaft. Damage to the compressor, including bent impeller blades and inlet guide vanes, was consistent with engine operation and the ingestion of material during the impact sequence.
The main rotor hub and blade assembly were located approximately 300 feet from the tower. One blade was found in its entirety, while the other was fractured mid-span. Both blades exhibited leading-edge gouges and striations consistent with the dimensions of the guy wire cabling.
A structural inspection of the tower following the accident revealed that the lower hardware for the top northwest guy wire was damaged, with bent tensioning rods and broken vibration dampers.
