The investigation

Mechanical examination of the flight controls and hydraulic servos revealed fractures and bending consistent with overload. The main rotor mast was found fractured at its base near the top of the transmission case, also showing features of overload separation. The engine, a Rolls-Royce M250-C30P, remained mounted but sustained fractured mounts and damage to the KAflex driveshaft. Damage to the compressor, including bent impeller blades and inlet guide vanes, was consistent with engine operation and the ingestion of material during the impact sequence.

The main rotor hub and blade assembly were located approximately 300 feet from the tower. One blade was found in its entirety, while the other was fractured mid-span. Both blades exhibited leading-edge gouges and striations consistent with the dimensions of the guy wire cabling.

A structural inspection of the tower following the accident revealed that the lower hardware for the top northwest guy wire was damaged, with bent tensioning rods and broken vibration dampers.
