What happened
On March 2, 2013, at 17:15 local time, a private Robinson R 4-4 II helicopter, registration EC-LIB, was performing a takeoff from an unpaved field near Clariana de Cardener, Lleida. The flight was a return leg of a private trip that had begun earlier that day from Sabadell.
During the takeoff maneuver, the aircraft lifted slightly but moved forward and laterally. As the helicopter gained altitude, a large cloud of dust was kicked up from the ground, completely enveloping the aircraft. This sudden loss of external visual references caused the pilot to lose control. The helicopter drifted toward nearby trees, and the pilot attempted to maneuver around them, resulting in a sharp turn to the right and a subsequent loss of stability. The aircraft impacted the ground on its left skid and overturned on its left side. All three occupants (the pilot and two passengers) were uninjured and able to exit the aircraft on their own.
The investigation
The CIAIAC examined the aircraft's condition, the flight parameters, and the environmental factors at the scene. Investigators noted that the takeoff occurred from an unpaved surface, which allowed for the formation of the dust cloud. The investigation also reviewed the aircraft's weight and balance, confirming that while the center of gravity was forward, it remained within established limits.
Witness statements from a passenger and a bystander confirmed the sudden appearance of the dust cloud. The investigation also considered the human factors involved, specifically the significant difference in experience between the pilot and the front passenger, noting a potential "authority gradient" effect where the pilot's decision-making might have been influenced by the presence of a much more experienced passenger.
