Boeing 767 Overruns Runway at Hamilton International Airport

A Cargojet Airways Limited Boeing 767-338 experienced a flap asymmetry warning and subsequent braking failure during landing at Hamilton, resulting in a runway overrun.

What happened

On 19 November 2024, a Boeing 767-338 cargo flight operated by Cargojet Airways Limited departed from John C. Munro Hamilton International Airport (CYHM) bound for Vancouver International Airport (CYVR). The aircraft was carrying two pilots and one jump seater.

During the descent phase of the flight, the flight crew identified an asymmetry flap warning. In response, the crew leveled the aircraft, informed air traffic control, and issued a PAN-PAN alert. To manage the flight following the slat issue, the crew performed the necessary checklists and prepared for an approach and landing at speeds higher than standard procedures.

Upon touchdown on Runway 08L, the crew attempted to use the brakes and deploy thrust reversers; however, the thrust reverse handles failed to engage. The aircraft subsequently overran the runway at approximately 120 knots, colliding with several approach lights. The aircraft exited the paved surface and sank into muddy terrain, coming to a stop roughly 1250 feet past the end of the runway. After declaring a MAYDAY and performing shutdown procedures, the occupants evacuated the aircraft with help from Air Rescue and Firefighting teams.

The investigation

The TSB is currently investigating the incident to determine the circumstances surrounding the mechanical failures and the runway excursion.
