What happened
On June 25, 2026, a Cargojet Airways Ltd. Boeing 767-328 departed Miami International Airport in the United States, heading toward Jorge Chávez International Airport in Peru. Prior to departure, the aircraft’s right-hand fuel totalizer was inoperative. Consequently, the crew verified fuel quantities using underwing measuring sticks in accordance with Minimum Equipment List procedures.
During the final approach, approximately 15 nautical miles from the runway, the flight crew received multiple alerts via the crew alerting system regarding both the right and left fuel pumps. Shortly after these warnings, the right engine failed. The pilots continued the approach and executed a smooth single-engine landing. The aircraft subsequently taxied to its assigned parking spot. Notably, the crew did not declare an emergency to air traffic control, nor did they inform controllers of the engine failure.
The investigation
Post-flight maintenance inspections revealed that the right main fuel tank was completely empty. Following refuelling and successful engine runs, the aircraft was returned to service. The Transportation Safety Board of Canada (TSB) is currently investigating the incident to determine why the fuel quantity monitoring systems failed to provide adequate warnings prior to the engine failure.
Findings
The investigation highlights a critical safety issue regarding fuel management when primary indication systems are inoperative. The reliance on manual verification methods, combined with the lack of emergency declaration despite an in-flight engine failure, presents significant operational concerns that require detailed analysis by the TSB.
