What happened
A scheduled flight operated by the carrier was traveling from Soeklam-Hatta Airport in Jakarta to Juanda Airport in Surabaya, East Java. The aircraft, a Boeing 737, was carrying a total of 155 persons, including 148 passengers and 7 crew members. During the transit, the flight crew reported no operational abnormalities. Upon arrival in Surabaya, the weather was characterized by rain and thunderstorms, with visibility measured at 8,000 meters and winds at 240/7 knots.
As the aircraft progressed on its final approach to runway 28, the crew identified the approach lights at approximately 800 feet and received landing clearance. Just before the aircraft made contact with the runway, the pilot in command took control from the co-pilot. During the landing sequence, the Ground Proximity Warning System issued "Sink Rate" and "Pull Up" alerts. The aircraft's right wheel track drifted approximately 4 meters off the runway surface before returning to the pavement. The aircraft eventually came to a halt roughly 100 meters from taxiway N3.
Following the touchdown, the fuselage sustained structural damage, specifically bending downward aft of the 16th passenger row. This caused panic among the occupants, prompting flight attendants to initiate an evacuation through all available exits using inflated door slides. The incident resulted in 2 minor injuries and caused severe damage to the airframe.
Findings
Data recovered from the Cockpit Voice Recorder (CVR) indicated that the crew engaged in discussions regarding company training programs and fuel policies until the aircraft reached 2,000 feet, rather than focusing on flight progress. Furthermore, the CVR showed a lack of formal approach briefings or the reading of necessary checklists during the descent and approach phases. The unstable approach was highlighted by the activation of GPWS warnings immediately preceding the excursion.
