What happened
On June 27, 2015, a pilot planned a flight in a Robinson R-44 II, registration RA-05765, from the vicinity of Kemerovo-Severny Airport to the Makarasky settlement. The flight was intended to carry one passenger. The flight plan had been submitted to the Novosibirsk Air Traffic Control Center in accordance with established procedures and received authorization.
At 01:05, the pilot and the passenger arrived at the departure point near Kemer_ovo-Severny Airport to begin pre-flight preparations. The passenger was the owner of a construction site located at the destination in Makarasky. While the flight plan was approved, the pilot did not request meteorological services from the Kemerovo AMC, though regulations allowed for the use of alternative weather information sources for decision-making.
The investigation
The investigation examined the pre-flight procedures and the pilot's compliance with aviation regulations. It was noted that the pilot did not undergo a formal pre-flight medical examination; however, under specific regulations (FAP-128), the pilot commander holds the authority to permit the crew to fly when a medical professional is unavailable at the departure site. The investigation also looked into the pilot's lack of direct meteorological consultation with the Kemerovo AMC.
