Brno-Tuřany Airport Accident: Flap Asymmetry and Loss of Control (HB-FOJ)

Brno-Tuřany, Czech RepublicLanding (descent or approach)

On May 26, 1998, a Pilatus PC-12 (registration HB-FOJ) operated by Adolf Henschel was involved in an aviation accident near Brno-Tuřany, Czech Republic during landing or approach. 2 people were killed. Investigators recorded the probable cause as: The critical situation was caused by flap asymmetry resulting from shear in torsion of the left-hand inner flexible shaft. Loss of controllability caused by reduction of airspeed with use of Beta range in the final phase of the flight was the direct cause. This summary draws on records from the Bureau of Aircraft Accidents Archives (B3A); 8 related events involving the same aircraft type or operator are linked below.

An aircraft crashed near Brno-Tuřany Airport after takeoff due to flap asymmetry and loss of control. Both occupants were killed.

Incident Overview and Radar Data After departing runway 28 at Brno-Tuřany Airport, the pilot requested to land and was instructed to continue a southern left-hand circuit. Secondary radar first detected the aircraft at approximately 520 feet above aerodrome level, 560 meters north of the runway. The pilot did not confirm the instruction and instead continued a northern right-hand turn. When the pilot failed to confirm repeated instructions for the left-hand circuit, the tower permitted the northern circuit and offered post-landing assistance. At this point, the aircraft was at approximately 930 feet AAL, 2.8 kilometers north of the aerodrome. The aircraft began a gradual descent and turn, likely intended for a runway 28 approach, but the approach was not completed. The aircraft struck the ground at 0615:28, approximately 600 meters north of the runway 28 threshold. Witnesses reported the aircraft was flying very low in the final phase, and the last secondary radar height was approximately 120 feet AAL, 13 seconds before impact. Witnesses described the aircraft's attitude before impact as very unusual. Both occupants were killed.

Probable cause

The critical situation was caused by flap asymmetry resulting from shear in torsion of the left-hand inner flexible shaft. Loss of controllability caused by reduction of airspeed with use of Beta range in the final phase of the flight was the direct cause. Incorrect application of emergency procedures for flap retraction listed in the Pilot's Operating Handbook was the main cause.
