Accident
While approaching Tinker Air Force Base at night, the flight crew encountered low visibility due to foggy conditions. The crew initiated a go-around maneuver and subsequently attempted four more approaches, each abandoned due to poor visibility. During the fifth approach, the crew declared an emergency because of low fuel. Shortly thereafter, the airplane stalled and crashed.
Occupants and Injuries
The accident resulted in one pilot fatality. The second occupant sustained serious injuries.
Probable Cause and Contributing Factors
According to the official investigation, the probable cause of the accident was a stall during a missed approach due to improper in-flight decisions. The following contributing factors were reported: improper IFR operation, failure to maintain flying speed, low ceiling, fog, zero visibility, low fuel, adverse/unfavorable weather, erratic flight path during the fifth attempted approach, and emergency declared due to low fuel.
