    A BLUE VAN ENTERED RUNWAY 16R VIA TAXIWAY 14-G, TRAVELED NORTH ON RUNWAY 16R TO ABEAM THE TOWER, WHERE IT TURNED AROUND AND TRAVELED SOUTH. A HAWKER H25A WAS ROTATING RUNWAY 16R AS THE VEHICLE ENTERED THE RUNWAY, APPROXIMATELY 1,500 TO 1,000 FEET FROM WHERE THE VEHICLE ENTERED. LOCAL CONTROL (LC) ADVISED THE H25A PILOT OF THE VEHICLE. THE H25A OVER FLEW THE VEHICLE BY 50 FEET AS REPORTED BY ATCT. THE VAN EXITED THE RUNWAY VIA TAXIWAY 13F AND AIRPORT POLICE AND OPERATIONS APPREHENDED THE DRIVER. THE H25A PILOT LATER STATED THEY RECEIVED A CALL FROM TOWER ABOUT VAN ENTERING THE RUNWAY AS THE AIRCRAFT WAS JUST BELOW V1/VR. CAPTAIN REPORTED NORMAL ROTATION AND ESTIMATED OVERFLYING VAN BY ABOUT 300 FEET.

    GROUND CONTROL (GC) APPROVED A TUG TOWING A MD-80 TO CROSS RUNWAY 35 VIA TAXIWAY D-4 TO TAXIWAY E-4 WITHOUT COORDINATING WITH LOCAL CONTROL (LC). LC CLEARED A MITSUBISHI MU2 FOR TAKE-OFF RUNWAY 35 ABEAM TAXIWAY KILO, INTERSECTION DEPARTURE. THE MU2 PILOT OBSERVED THE TUG PULL ONTO RUNWAY 35 WHILE ON DEPARTURE ROLL. THE MU2 PILOT VEERED LEFT, ABORTING TAKEOFF TO AVOID HITTING THE TUG BUT CLIPPED HIS RIGHT WING TIP ON THE TUG AND ENDED UP OFF THE RUNWAY. THE MU2 PILOT ADVISED TOWER THEY IMPACTED THE TUG. THIS COLLISION RESULTED IN DAMAGE BUT NO INJURIES. KILO IS 1,950 FEET FROM TAXIWAY E-4. AMASS ALARMED WHEN THE MU2 WAS 100 FEET FROM TAXIWAY E-4.

    LOCAL CONTROL (LC) INSTRUCTED A CESSNA C152 TO TAXI INTO POSITION AND HOLD (TIPH) RUNWAY 1 AT A1 (APPROACH END), FOR AN ARRIVAL TO CLEAR THE RUNWAY. WHILE WAITING FOR TRAFFIC TO CLEAR THE RUNWAY A BEECH BE36 REPORTED 2 MILE RIGHT BASE TO RUNWAY 1. LC CLEARED THE BE36 TO LAND ANTICIPATING THE C152 WOULD BE CLEAR OF THE RUNWAY. LC DID NOT GIVE TRAFFIC INFORMATION FOR EITHER THE C152 OR BE36. BEFORE LC COULD CLEAR THE C152 FOR TAKEOFF THERE WERE TWO EXCHANGES OF TRANSMISSIONS WITH A HELICOPTER REPOSITIONING AND A CESSNA REQUESTING A FREQUENCY CHANGE. LC DID NOT CLEAR THE C152 FOR TAKEOFF AND THE BE36 LANDED OVER THE CESSNA HOLDING IN POSITION. CLOSEST VERTICAL SEPARATION REPORTED WAS 300 FEET. (DISPLACED THRESHOLD IS 700 FEET FROM APPROACH END)

    A CESSNA C172 WAS CLEARED TOUCH AND GO RUNWAY 27L WITH A CESSNA C182 ON A 2 MILE FINAL AND CLEARED TO LAND ON INTERSECTING RUNWAY 17. THE C172 LANDED RUNWAY 27L AND APPEARED TO ALMOST STOP ON THE RUNWAY. THE C182 WAS AT THIS TIME ONE QUARTER (.25) MILE FINAL RUNWAY 17. LOCAL CONTROL (LC) INSTRUCTED THE C172 TO TURN LEFT AND EXIT RUNWAY 27L AT TAXIWAY BRAVO. THE C172 DID NOT ACKNOWLEDGE AND CONTINUED INTO THE INTERSECTION OF RUNWAYS 27L AND 17. LC ISSUED 3 INSTRUCTIONS FOR THE C172 TO HOLD POSITION AND THE C172 DID NOT COMPLY. THE C182 LANDED RUNWAY 17 AND PASSED 75 FEET BEHIND THE C172. THE C172 PILOT LATER ADVISED TOWER THAT HE HAD A FLAT TIRE.

    A DOUGLAS DC8 WAS INSTRUCTED TO HOLD SHORT RUNWAY 13R VIA TAXIWAY A-7. THE DC8 PILOT READ BACK INSTRUCTIONS BUT CROSSED RUNWAY 13R AT A7 WITHOUT CLEARANCE AND CONFLICTED WITH A BOEING B757 ON TAKEOFF ROLL SAME RUNWAY. THE B757 ABORTED TAKEOFF. CLOSEST HORIZONTAL SEPARATION REPORTED WAS 200 FEET. THE CAPTAIN OF THE B757 STATED THEY HAD ABORTED AT AROUND 110 KIAS USING MAX BRAKING. THE PRELIMINARY SEPARATION REPORTED BY THE CAPTAIN WAS 100 FEET.

    A SWEARINGEN SW4 LANDED ON RUNWAY 18R AND WAS INSTRUCTED TO TURN OFF VIA TAXIWAY JULIET BUT WAS NOT ADVISED OF ANY CROSSING TRAFFIC. THE SW4 ACKNOWLEDGED INSTRUCTIONS FOR JULIET. LC THEN ISSUED A CANADAIR CRJ2, WHO HAD BEEN HOLDING IN POSITION ON INTERSECTING RUNWAY 27, A TAKEOFF CLEARANCE WITHOUT INSURING THE SW4 WAS CLEAR OF RUNWAY 18R. THE SW4 ADVISED LC THEY MISSED JULIET (LOCAL TIME WAS 00:25, VISIBILITY ONE QUARTER (.25) MILE IN DRIZZLE, SIGNAGE LIGHTS FOR JULIET AND KILO WERE NOT WORKING). LC INSTRUCTED THE SW4 TO STOP BUT THE AIRCRAFT HAD ALREADY ENTERED THE INTERSECTION OF RUNWAY 27/18R. THE CRJ2 SAW THE SW4 ENTER RUNWAY 27, ABORTED THEIR TAKE-OFF, APPLIED HEAVY BRAKING, AND MANEUVERED TO GO BEHIND THE SW4. CLOSEST PROXIMITY WAS REPORTED AS 30 FEET LATERAL. CRJ2 PILOT SAYS BRAKING USED WAS 10 ON A SCALE OF 1 TO 10, MOVED TO THE RIGHT TO MISS, PASSED THE SW4 AT 30-40 KTS, AND CLEARED BY 20 (NOT 30) FEET.

TWO AIRCRAFT WERE ON FINAL AND THE FARTHEST ONE OUT, A CESSNA, WENT AROUND. TOWER DID NOT REALIZE THIS AND A CESSNA C152, THE CLOSEST AIRCRAFT TO RUNWAY 29, LANDED 300 FEET BEHIND THE DEPARTING PA23.

    A MCDONNELL-DOUGLAS MD80 LANDED RUNWAY 4R AND REPORTED THEY “ALMOST HIT A PEDESTRIAN CROSSING IN FRONT OF THEM”. THE CAPTAIN REPORTED THAT HE SAW THE PEDESTRIAN JUST PRIOR TO LANDING AND THE PEDESTRIAN APPEARED TO SCURRY OUT OF THE WAY, AND AS THE NOSE WHEEL OF THE AIRCRAFT WAS TOUCHING DOWN, THE CAPTAIN BELIEVED THE LEFT WING PASSED OVER THE INDIVIDUAL. AIRSPEED WAS ESTIMATED AT APPROXIMATELY 120 KIAS. THE UNAUTHORIZED INDIVIDUAL JUMPED THE FENCE FROM THE TURNPIKE AND WAS APPREHENDED BY SECURITY.
