    LOCAL CONTROL (LC) AUTHORIZED GROUND CONTROL (GC) TO PROCEED ONTO RUNWAY 19L WITH AIRPORT VEHICLES. LC DID NOT PLACE "RUNWAY CLOSED" MEMORY JOGGER STRIPS IN APPROPRIATE BAYS. A LEARJET LJ35 WAS CLEARED FOR THE OPTION ON RUNWAY 19L. THE LJ35 COMPLETED A TOUCH AND GO WHILE AIRPORT VEHICLE WAS TRAVELING SOUTHBOUND ON RUNWAY 19L AND 1,500 FEET FROM DEPARTURE END. IN ADDITION, A BARRIER MAINTENANCE VEHICLE WAS CLEARING THE RUNWAY EASTBOUND AT THE BAK-12 CABLE, 1,500 FEET FROM THE DEPARTURE END. THE LJ35 TOUCHED DOWN BY E3, ROTATING PRIOR TO E4, AND FLEW OVER AIRPORT VEHICLE NEARING BAK-12 WITH CLOSEST PROXIMITY REPORTED AS 100 FEET VERTICAL. DISTANCE FROM E4 TO BAK-12 IS APPROXIMATELY 1,200 FEET.

    A CESSNA C150 WAS CLEARED FOR TAKEOFF ON RUNWAY 29R ABEAM TAXIWAYT B2 WITH A SWEEPER ON THE RUNWAY BETWEEN B9 AND B10. THE C150 OVER FLEW THE SWEEPER DURING CLIMB OUT BY AN ESTIMATED 250 FEET VERTICAL. LC DID NOT SEE WHERE THE C150 ROTATED. PILOT STATED IN FOLLOW-UP PHONE CONVERSATION THAT HE ROTATED NEAR B6. DISTANCE FROM B6 TO B9 IS APPROXIMATELY 1,500 FEET. B2 TO B9 IS 3500 FEET. MEMORY AID WAS IN PLACE OVER THE LC WIND INDICATOR TO SHOW THE RUNWAY WAS OCCUPIED BUT LC LOOKED AT THE WIND INDICATOR AT GC INSTEAD AND FORGOT THE SWEEPER WHO HAD BEEN ON THE RUNWAY FOR 19 MINUTES.

    A GULFSTREAM GLF5 WAS INSTRUCTED TO CROSS RUNWAY 25L AT GOLF AND HOLD SHORT OF RUNWAY 25R. THE GLF5 READ BACK HOLD SHORT THEN PROCEEDED TO CROSS 25R WITHOUT CLEARANCE AND CONFLICTED WITH A CANADAIR CRJ7 ON DEPARTURE 25R FROM TAXIWAT FOXTROT. THE CRJ7 ABORTED AND STOPPED APPROXIMATELY 100 FEET FROM GOLF AS THE GLF5 WAS HALFWAY ACROSS THE RUNWAY. DISTANCE FROM FOXTROT TO GOLF IS 3250 FEET. PILOT OF CRJ7 REPORTED BRAKES NEEDED A 15 MINUTE COOL DOWN. THIS EVENT MET THE PARAMETERS ESTABLISHED WITHIN AMASS TO ALARM SO AN ALERT WAS GIVEN. INCIDENT OCCURRED AT 1802L ON 9/30/06 BUT REPORT FORM USED ZULU REPORT DATE OF 10/1/06.

    A AERONCA CH7A HAD BEEN CLEARED FOR TAKEOFF RUNWAY 17L AT A3 (DISPLACED THRESHOLD AT APPROACH END) WHEN A PIPER PA28A LANDED SAME RUNWAY WITHOUT AUTHORIZATION OR CONTACT WITH WDG ATCT. THE CH7AS TAKEOFF CLEARANCE WAS CANCELLED AND ATCT STOPPED THE CH7A JUST SHORT OF THE RUNWAY EDGE ON A3. ATCT ATTEMPTED TO CONTACT THE PA28A, ISSUED GO AROUND PRIOR TO ONE (1) MILE AND GAVE A RED LIGHT ALL THE WAY TO THE RUNWAY WITHOUT SUCCESS. THE PA28A LANDED APPROXIMATELY 200 FEET PRIOR TO THE DISPLACED THRESHOLD FOR 17L AND ROLLED PAST THE CH7AS POSITION BY APPROXIMATELY 20-25 FEET HORIZONTAL.

    A CESSNA C172 WAS TAXIED TO RUNWAY 13 VIA TAXIWAY ALPHA. THE C172 TURNED OFF ALPHA ONTO ALPHA 3 WITHOUT AUTHORIZATION, CROSSED THE HOLD LINES, AND CAME TO A STOP AT THE RUNWAY EDGE OF THE INTERSECTION OF RUNWAYS 13/31 AND 18/36 AND CONFLICTED WITH A PIPER PA28A ON A TOUCH AND GO FOR RUNWAY 13 AT THE TIME. THE PA28A ROTATED APPROXIMATELY 500 FEET FROM 13/18 INTERSECTION THEN PASSED THE INTERSECTION AT AN ESTIMATED 50 FEET IN A CLIMB. IN ADDITION, A ROBINSON R22, LESS THAN A MILE FINAL WAS ALSO ISSUED A GO AROUND.

    LOCAL PROCEDURES GIVE CONTROL OF RUNWAY 7/25 TO GROUND CONTROL (GC). LOCAL CONTROL (LC) REQUESTED CONTROL OF RUNWAY 07/25 FOR LANDING TRAFFIC AND GC RELEASED CONTROL. A BEECH BE55 WAS TAXIED FROM JET WEST AVIATION TO RUNWAY 16R VIA TAXIWAY ALPHA, KILO, AND BRAVO BY GC WHO FORGOT LC HAD CONTROL OF 7/25. TAXIWAY ALPHA CROSSES RUNWAY 7. LC SCANNED RUNWAY 7 WHEN A CESSNA C210 WAS ON SHORT FINAL AND NOTICED THE BE55 WELL SHORT OF THE HOLD SHORT MARKINGS IN A SLOW TAXI BELIEVING THE AIRCRAFT WAS GOING TO HOLD SHORT. LC CONTINUED TO SCAN TO THE END OF RUNWAY 7 BEFORE TURNING BACK TO NOTE THE PROGRESS OF THE C210. WHEN LC DID, HE NOTED THAT THE C210 WAS ON LANDING ROLL AND THE BE55 WAS CROSSING THE RUNWAY. TAXIWAY ALPHA IS APPROXIMATELY 2,000 FEET FROM APPROACH END RUNWAY 7. THE C210 USED HEAVY BRAKING AND SWERVED BEHIND THE BE55 TO AVOID THE AIRCRAFT. CLOSEST PROXIMITY REPORTED WAS 20 FEET HORIZONTAL.

    A CESSNA C310, ON ILS APPROACH RUNWAY 9R, WAS INSTRUCTED BY APPROACH CONTROL TO CONTACT PTK TOWER. THE C310 FAILED TO CONTACT TOWER. LOCAL CONTROL (LC) HAD JUST CLEARED A PIAGGIO P180 INTO POSITION AND HOLD (TIPH) RUNWAY 9R AT TAXIWAY MIKE WHEN THE C310 WAS OBSERVED ON ONE HALF (.50) MILE FINAL 9R. LC ATTEMPTED TO SEND THE C310 AROUND THEN INSTRUCTED THE P180 TO STOP, WHICH PILOT DID WITH NOSE WHEEL OVER THE RUNWAY EDGE LINE AND AS THE C310 LANDED PAST MIKE. CLOSEST PROXIMITY ESTIMATED AS 40 TO 75 FEET LATERAL, AND 75 TO 100 FEET VERTICAL (ALTITUDE OF C310 AS IT PAST MIKE).

    A BOEING B737 WAS CLEARED FOR TAKEOFF RUNWAY 6 FULL LENGTH. AIRPORT OPERATIONS REQUESTED AND WAS GRANTED PERMISSION TO INTERCEPT AN UNAUTHORIZED VEHICLE ON TAXIWAY ALPHA THAT HAD CRASHED THROUGH THE UPS GATE. THE VEHICLE DROVE AT HIGH SPEED VIA ALPHA TO A4 AND TURNED ON A4 TOWARDS RUNWAY 6. AT THIS POINT, LOCAL CONTROL WARNED THE B737 OF THE VEHICLE RAPIDLY APPROACHING THE RUNWAY. THE VEHICLE TURNED LEFT ON RUNWAY 6 EASTBOUND AND HUGGED THE GRASS LINE PARALLELING THE RUNWAY. THE B737 PILOT SAW THE VEHICLE AND ROTATED AT A4 THEN PASSED THE VEHICLE AT TAXIWAY A5. CLOSEST PROXIMITY AS THE B737 PASSED THE VEHICLE WAS 100 FEET VERTICAL AND 30 FEET HORIZONTAL.
