    A CESSNA C210 WAS CLEARED FOR TAKEOFF ON RUNWAY 17R ABEAM TAXIWAY CHARLIE. A PIPER P46T FAILED TO HOLD SHORT OF RUNWAY 17R AND TAXIED ONTO THE RUNWAY AT TAXIWAY ECHO EASTBOUND. LOCAL (LC) CANCELLED THE C210’S TAKEOFF CLEARANCE WHEN THE AIRCRAFT WAS ESTIMATED TO BE MIDPOINT OF CHARLIE AND ECHO (NEAR DELTA AND APPROXIMATELY 900 FEET FROM ECHO). THE C210 PASSED ECHO INTERSECTION WHERE THE P46T WAS STOPPED ON THE WEST SIDE OF RUNWAY BETWEEN RUNWAY EDGE AND RUNWAY CENTERLINE (RUNWAY WIDTH IS 100 FEET). THE C210 PILOT ADVISED HE STAYED ON THE LEFT SIDE OF THE RUNWAY TO AVOID THE P46T AND ESTIMATED THE DISTANCE TO BE BETWEEN 40-50 FEET LATERAL. CLOSEST HORIZONTAL PROXIMITY REPORTED WAS LESS THAN 100 FEET.

    LOCAL (LC) CLEARED A BOEING B737 FOR DEPARTURE ON RUNWAY 12R FULL LENGTH AND THEN CLEARED A BELL 06 (206 HELICOPTER) VFR FOR NORTH DEPARTURE (COUGAR DEPARTURE) FROM THE SOUTH RAMP HELICOPTER PAD WITH NO RESTRICTIONS. NORMALLY HELICOPTERS DEPART TOWARDS THE ARRIVAL END OF RUNWAY 4 AND TURN NORTH OVER MID-FIELD AND CONTINUE NORTHBOUND. THE B06 DEPARTED TOWARDS THE DEPARTURE END OF RUNWAY 12R, TURNED SOUTHEAST CROSSING OVER TAXIWAY NOVEMBER THEN TURNED NORTH PASSING QUEBEC AND STARTED TO AND CROSS RUNWAY 12R WHEN PILOT OBSERVED THE B737 AND TURNED BACK TOWARDS THE SOUTHWEST. ASDE-X ANALYSIS INDICATED THE CLOSEST PROXIMITY OF THE TWO AIRCRAFT WAS WHEN THE B06 WAS APPROXIMATELY 65 FEET FROM RUNWAY EDGE AT AN ESTIMATED 275 FEET VERTICAL AND 115 FEET FROM THE B737 WHO WAS AT AN ESTIMATED 100 FEET VERTICAL. THE B737 SAW THE B06 AND LEVELED OFF TO STAY BELOW THE HELICOPTER. A NMAC WAS FILED BY B737 PILOT ON 4/28/10. CLOSEST PROXIMITY REPORTED 100 FEET VERTICALLY AND 125 FEET LATERALLY.

    A CESSNA C208 WAS CLEARED TO LAND ON RUNWAY 25L WHICH PILOT READ BACK. A BOEING B737 WAS IN POSITION ON PARALLEL RUNWAY 25R FULL LENGTH (E13) WITH CLEARANCE FOR TAKEOFF. THE C208 THEN FLEW OVER THE B737 AND LANDED ON RUNWAY 25R INSTEAD OF 25L AS CLEARED. THE B737 OBSERVED THE C208 AND HELD. CLOSEST PROXIMITY REPORTED WAS 50-100 FEET VERTICAL. AVI ANALYSIS INDICATED THE C208 FLEW JUST LEFT OF THE B737 AT AN ESTIMATED 125 FEET AGL AND ENTERED THE RUNWAY AREA 500 FEET IN FRONT OF B737, LANDED THEN EXITING AT TAXIWAY F6. ASDE-X DID NOT ALARM.

    AN AIRPORT MAINTENANCE VEHICLE, IN PURSUIT OF AN ANIMAL, ENTERED RUNWAY 35 AT TAXIWAY CHARLIE WITHOUT AUTHORIZATION AND CONFLICTED WITH A CESSNA C560 THAT HAD LANDED SAME RUNWAY. THE C560 THEN ROTATED AND BECAME AIRBORNE NEAR THE INTERSECTION OF RUNWAY 35 AND RUNWAY 30 TO AVOID THE VEHICLE AND ANIMAL. AS REPORTED BY THE NON-FLYING PILOT: THEY RECOGNIZED THE CONFLICT JUST BEFORE TOUCHDOWN AND AGGRESSIVELY ROTATED WITH THE NON-FLYING PILOT APPLYING ADDITIONAL BACKPRESSURE TO THE CONTROLS. THE ROTATION WAS EARLY AND RESULTED IN A MOMENTARY STALL WARNING. THE MAIN GEAR TOUCHED DOWN BEFORE THEY BECAME AIRBORNE AGAIN. A DIRECT OVERFLIGHT OCCURRED AS THE TRUCK CROSSED THE RUNWAY IN PURSUIT OF THE ANTELOPE. THE CLOSEST VERTICAL PROXIMITY WAS ESTIMATED AT 10 - 20 FEET. CLOSEST PROXIMITY REPORTED WAS 50 FEET VERTICAL. DISTANCE FROM INTERSECTION OF RUNWAY 35/30 TO CHARLIE IS APPROXIMATELY 2,000 FEET.

    A PIPER PA22 WAS CLEARED TO LAND ON RUNWAY 8R BUT INSTEAD LANDED ON INTERSECTING RUNWAY 3 WITHOUT CLEARANCE. THE PA22 CONTINUED DOWN THE RUNWAY THROUGH THE RUNWAY 8L INTERSECTION AND UNDER A AEROSPATIALE AS50 (HELICOPTER) CLEARED FOR THE OPTION ON RUNWAY 8L. THE AS50 LANDED LONG AND APPROXIMATELY 100 FEET PAST THE RUNWAY 8L/3 INTERSECTION OVERFLYING THE PA22 BY 50 FEET VERTICAL, THE CLOSEST PROXIMITY REPORTED. THE AS50 PILOT EXECUTED A TOUCH AND GO AND DID NOT SEE THE PA22.
