What happened
On July 28, 2011, a TEXAS HELICOPTER CORP OH-13H/M74A, registration N10009, was being landed on a helicopter cart in Creston, Illinois. During the landing sequence, the helicopter's skid became caught on the cart, causing the aircraft to roll over. The incident resulted in 1 minor injury and 0 fatalities.
The investigation
The investigation determined that the aircraft sustained substantial damage to its tailboom during the rollover. The pilot reported that there were no mechanical malfunctions present during the flight. The analysis noted that the event was a dynamic rollover, which occurs when a helicopter continues to roll after exceeding a critical angle while one skid is fixed to the ground, such as when a skid is caught by a projection or when landing on a slope.
