Incident Overview
On May 8, 2013, at Warsaw Chopin Airport (EPWA), an Embraer 170 aircraft was taxiing for takeoff. The aircraft was proceeding on Taxiway O toward Taxiway A when a Mulag ground vehicle, traveling on Taxiway A, crossed the aircraft's path. The pilot of the Embraer 170 applied brakes and subsequently informed the tower controller that the vehicle driver was using a mobile phone during the incident.
Investigation and Findings
The Polish State Commission on Aircraft Accident Investigation (PKBWL) oversaw the investigation. The vehicle operator, LS Airport Services, was required to submit a report on the incident. Despite repeated requests, LS Airport Services did not provide the report to PKBWL. As a result, PKBWL ended its supervision of the incident investigation.
PKBWL determined that the most probable cause of the incident was the vehicle operator's failure to maintain proper attention and non-compliance with the operating procedures applicable at EPWA airport.
Safety Recommendations and Preventive Measures
No preventive measures were reported because LS Airport Services did not send the incident investigation report to PKBWL. The Commission did not issue any safety recommendations.
