Fatal training accident involving single-engine shutdown at Samarkand Airport

5 fatalities • Samarkand, Uzbekistan • Takeoff (climb)

A training flight involving an engine shutdown procedure resulted in a fatal crash near Samarkand Airport, claiming the lives of all five crew members.

What happened

During a scheduled local training session consisting of various approaches and touch-and-go maneuvers, the crew departed from Samarkand Airport at 06:15 local time. After successfully completing eight circuits, the flight was on its ninth approach when the instructor performed an engine shutdown on the right engine and feathered the propeller. During this maneuver, the flaps were extended to 38 degrees.

As the aircraft passed approximately 10 meters above the runway at a speed of 190 km/h, the crew attempted to execute a go-around near the end of the runway. Upon applying power, the aircraft entered a nose-up attitude and reached a super critical angle of attack. The plane drifted slightly to the right before descending and impacting the ground in flames 1,120 meters beyond the runway threshold. The wreckage was located 690 meters to the right of the extended centerline. The impact and subsequent fire resulted in five fatalities among the crew.

Findings

Investigations into the accident identified several contributing factors related to the crew's actions during the go-around. It was determined that the crew failed to maintain sufficient airspeed for the maneuver, largely because they took approximately 10 seconds too long to retract the landing gear and flaps. Additionally, the training syllabus did not specifically include procedures for an engine shutdown during an approach, and the instructor was performing this specific procedure for the first time.

Probable cause

The accident was caused by the crew's failure to maintain adequate airspeed during a go-around due to delayed retraction of the flaps and landing gear.
