What happened
On 20 March 2021, a Boeing 737-400F, registration PK-YSF, was performing a non-scheduled cargo flight from Halim Perdanakusuma International Airport in Jakarta, Indonesia, destined for Sultan Hasanuddin International Airport. The flight crew consisted of two pilots, one engineer, and one flight operation officer. The aircraft departed at 10:52:57 LT following a takeoff with reduced thrust.
Shortly after becoming airborne, the pilot monitoring reported a failure of the right engine. The crew requested to proceed to the AL NDB, and the air traffic controller provided instructions to climb to 2,500 feet and maintain an outbound heading. After holding for approximately 15 minutes, the crew prepared for an approach to Runway 24.
At 11:25 LT, the crew reported that the runway was in sight and received landing clearance. Upon touching down in the touchdown zone, the right main landing gear experienced a structural failure, causing both wheels to detach from the aircraft. Following the impact, the air traffic controller observed sparks and an engine fire. The crew confirmed that fire was visible on the left side of the aircraft during the incident.
