What happened
During an approach to Amsterdam-Schiphol Airport following a flight from New York via Shannon, the crew of the Willem Bontekoe, a four-engine aircraft, was cleared for a series of descents. After being authorized to descend through 5,500 feet, 4,500 feet, and 3,500 feet, the aircraft vanished from radar at 11:30 local time after impacting the sea.
Air traffic controllers attempted to contact the crew with a clearance to descend to 2,500 feet approximately five minutes after the disappearance, but no response was received. Search and rescue operations were significantly obstructed by adverse weather conditions. Debris from the wreckage was eventually located at 16:10 local time, roughly 37 km northwest of Ijmuiden. The accident resulted in 21 fatalities, with no survivors reported. Less than half of the aircraft's structure was recovered from the ocean.
Findings
Following a 15-month investigation, authorities were unable to establish the definitive cause of the crash. Investigators explored several hypotheses, including an electrical system overheat leading to heavy smoke, an explosion involving high-pressure bottles, a failure of a cockpit window, or a malfunction of the autopilot system. However, none of these theories could be reconciled with all available evidence and occurrences, leaving the exact reason for the impact undetermined.
