    REPORTED WEATHER WAS ONE QUARTER (.25) SM FOG AND RUNWAY 9 RVR AT 1,600. AIRPORT VEHICLE REQUESTED CLEARANCE ON RUNWAY 9 FOR LIGHT MAINTENANCE. THIS WAS APPROVED AND LOCAL CONTROL (LC) ACTIVATED THE RED RUNWAY OCCUPIED LIGHT. APPROXIMATELY 29 MINUTES LATER, LC ISSUED TAKEOFF CLEARANCE RUNWAY 9 FULL LENGTH TO A CESSNA C750 FORGETTING ABOUT THE VEHICLE. RUNWAY 9 WAS NOT VISIBLE TO THE TOWER DUE TO FOG. PILOT OF C750 REPORTED PASSING A VEHICLE ON THE RUNWAY BY THE EDGE LIGHTS NEAR TAXIWAY H1, ABOUT 1,500 FEET FROM THE APPROACH END. LC CANCELLED THE TAKEOFF CLEARANCE BUT THE C750 WAS ALREADY AIRBORNE. CLOSEST HORIZONTAL PROXIMITY REPORTED WAS 65 FEET ON THE SURFACE OF RUNWAY 9.

    RUNWAY 16/34 IS CLOSED AND UNDER CONSTRUCTION. TAXIWAY A HAS BEEN TEMPORARILY CONVERTED INTO RUNWAY 17/35. A CONSTRUCTION VEHICLE (CHEVY PICK-UP TRUCK) ORIGINATING FROM THE CLOSED RUNWAY, PROCEEDED VIA TAXIWAY DELTA TO RUNWAY 17 WITHOUT ANY COMMUNICATIONS WITH THE TOWER. ACCORDING TO THE CONTROLLER THE VEHICLE ENTERED THE RUNWAY AT HIGH SPEED, AS A BEECH BE99 WAS JUST PASSING TAXIWAY D ON LANDING ROLL OUT RUNWAY 17. THE CONTROLLER STATED THAT IT JUST MISSED THE TAIL OF THE AIRCRAFT AND THE VEHICLE TURNED NORTHBOUND ON THE RUNWAY. ACCORDING TO IFP AIRPORT MGR, THE INTERSECTION IS PROPERLY SIGNED AND MARKED AND THE CONTRACTOR INFORMED HIM THAT ALL, TO INCLUDE THE SUBCONTRACTORS WERE TRAINED.

    A CESSNA C172 WAS TAXIED TO RUNWAY 17L TO HOLD SHORT OF RUNWAY 9 AT TAXIWAY ALPHA. A REVIEW OF THE TAPES SHOWED THE C172 DID READ BACK THE INSTRUCTIONS CORRECTLY. GROUND CONTROL (GC) OBSERVED THE C172 IN THE MIDDLE OF RUNWAY 9 NORTHBOUND AND ADVISED LOCAL CONTROL (LC). A BEECH BE33 WAS LANDING RUNWAY 9 AND WHEN LC WAS TOLD OF THE CONFLICT BUT IT WAS TOO LATE TO SEND THE BE33 AROUND. GC DID TRY TO HAVE THE C172 CONTINUE TO TAXI AND CLEAR THE RUNWAY. THE BE33 REPORTED HE DID TAKE EVASIVE ACTION BY SWERVING BEHIND THE C172 AND REPORTED CLOSEST HORIZONTAL PROXIMITY OF 20 FEET.

    AN EMBRAER E145 TAXIING TO RUNWAY 29 FOR DEPARTURE WAS INSTRUCTED TO HOLD SHORT OF RUNWAY 22L APPROACH PATH ON TAXIWAY ZULU. THE E145 READ BACK HOLD SHORT INSTRUCTIONS CORRECTLY THEN CROSSED THE APPROACH PATH OF RUNWAY 22L WITHOUT CLEARANCE AND CONFLICTED WITH A BOEING B737 PASSING OVER ZULU ON APPROACH FOR 22L. CLOSEST PROXIMITY REPORTED WAS 400 FEET VERTICAL AS THE B737 PASSES OVER THE E145. THE AMASS LOGS RECORDED AN ALTITUDE OF 200 FEET FOR THE B737 AS IT CROSSED OVER THE E145 ON ZULU. IT SHOULD BE NOTED THAT THE ALTITUDE RECORDED BY AMASS (ORIGINATING FROM THE AIRCRAFT ALTIMETER) HAS AN ACCURACY OF +/- ONE HUNDRED FEET.

    AN AIRBUS A320 WAS CLEARED TO LAND RUNWAY 10 AND WAS APPROXIMATELY 6 MILE FINAL WHEN LOCAL CONTROL (LC) CLEARED A CANADAIR CRJ1 FOR TAKEOFF ON INTERSECTING RUNWAY 15R FULL LENGTH. THE CRJ1 WAS TAXIING ON TAXIWAY PAPA ABOUT 500 FEET SHORT OF RUNWAY 15R WHEN TAKEOFF CLEARANCE WAS ISSUED. THE CRJ1 ROTATED AT AN ESTIMATED 3,500 FROM DEPARTURE POINT, NEAR TAXIWAY FOXTROT, APPROXIMATELY 1,800 FEET FROM 10/15R INTERSECTION AND FLEW PAST THE INTERSECTION BY AN ESTIMATED 300 FEET VERTICAL AS THE A320 WAS ENTERING RUNWAY 10/15R INTERSECTION. AMASS ANALYSIS STATES THE LATERAL DISTANCE BETWEEN THE TWO AIRCRAFT AS THE DEPARTURE WAS IN THE CENTER OF INTERSECTION 10/15R AND THE A320 WAS ROLLING OUT WAS 480 FEET. AT THIS POINT, THE DEPARTING AIRCRAFT WAS STILL WITHIN RADAR COVERAGE, WHICH IS APPROXIMATELY 200 FEET ABOVE THE SURFACE. AWE 83 EXITED RUNWAY 10 AT RUNWAY 22, WHICH IS A NORMAL EXIT POINT. THIS EVENT MET THE PARAMETERS ESTABLISHED WITHIN AMASS TO CAUSE AN ALARM AND AN ALERT WAS RECEIVED.

THE C182 THEN CROSSED 7L AT N5 WITHOUT CLEARANCE AND CONFLICTED WITH A BEECH BE20 ON DEPARTURE ROLL 7L FROM ABEAM P2, APPROXIMATELY 3,000 FEET WEST OF N5. ATCT ADVISED THE C182 OF THE IMPROPER CROSSING AND THE PILOT MADE A 180 TO EXIT. THE BE20 CONTINUED DEPARTURE ROLL AND AS IT PASSED N5 AND THE C182 WAS JUST ABOUT OFF THE RUNWAY. THE BE20 STATED HE WAS COMMITTED TO DEPART WHEN HE SAW THE C182. THE C182 PILOT SAID HIS BRAKES STUCK. CLOSEST HORIZONTAL PROXIMITY REPORTED WAS 70 FEET.

    A PIPER PA24 LANDED RUNWAY 26R AND WAS INSTRUCTED TO TURN LEFT AT BRAVO AND HOLD SHORT RUNWAY 26L. THE PA24 DID NOT READ BACK THE HOLD SHORT INSTRUCTION AND LC DID NOT HAVE PILOT READ BACK THE HOLD SHORT AND THE PA24 CONTINUED HIS TAXI CROSSING THE DEPARTURE END OF RUNWAY 26L ON BRAVO. A CESSNA C172 ON A TOUCH AND GO 26L, ROTATED ABEAM THE CONTROL TOWER AND PASSED OVER THE PA24 BY APPROXIMATELY 30 FEET VERTICAL IN A CLIMB. IN ADDITION, AFTER THE PA24 CROSSED THE RUNWAY THE PILOT CONTINUED TAXI WESTBOUND ON ALPHA WITHOUT CONTACTING GROUND CONTROL (GC). GC HAD OTHER AIRCRAFT TAXIING AND INSTRUCTED THEM TO HOLD POSITION UNTIL GC WAS ABLE TO CONTACT THE PA24. ONCE CONTACT WAS MADE, GC INSTRUCTED THE PA24 PILOT TO HOLD PRESENT POSITION.

ATCT CLEARED THE BE58 TO LAND 2L AND TRAFFIC GIVEN FOR AIRCRAFT LANDING ON PARALLEL. THE BE58 INSTEAD LANDED RUNWAY 2R AND CONFLICTED WITH A PIPER PA28A STILL ON LANDING ROLLOUT SAME RUNWAY. THE BE58 APPEARED TO SWERVE TO AVOID THE PA28A EXITING THE RUNWAY AT TAXIWAY DELTA. CLOSEST PROXIMITY ESTIMATED LESS THAN 50 FEET.
