LC ADVISED TRUCK TO PROCEED ON TAXIWAY BRAVO 2 AND RUNWAY 29R. FIVE MINUTES LATER, C525 CALLED INBOUND ON THE ILS 29R AND WAS CLEARED TO LAND. FIVE MINUTES AFTER RECEIVING LANDING CLEARANCE, C525 OVERFLEW TRUCK WHILE LANDING RUNWAY 29R. LOSS OF SEPARATION, CLOSEST PROXIMITY WAS 50 FEET VERTICAL.

    LOCAL CONTROL (LC) CLEARED C550 TO LAND RUNWAY 35 APPROXIMATELY FOUR MINUTES OUT. WHEN THE C550 WAS ON A ONE-MILE FINAL, LC CLEARED AN AIRPORT VEHICLE ONTO RUNWAY 35. VEHICLE PARKED ON THE RUNWAY THRESHOLD MARKINGS APPROXIMATELY 170 FEET FROM THE END AND 40 FEET WEST OF THE CENTERLINE. THE DRIVER EXITED THE VEHICLE TO WORK ON A RUNWAY EDGE LIGHT. VEHICLE DRIVER LATER REPORTED THE C550 OVERFLEW THE VEHICLE AT APPROXIMATELY 40 FEET AGL. C550 WINGSPAN IS 52 FEET, WHICH WOULD HAVE PLACED THE WINGTIP 14 FEET LATERALLY FROM THE VEHICLE IF THE PILOT FLEW THE CENTERLINE. THERE WERE NO REPORTS OF EVASIVE ACTION. LC DID NOT DETECT OR SEE THE EVENT. VISUAL RID WAS ACTIVATED BUT WITHOUT THE AURAL ALARM.

RUNWAY 24 WAS CLOSED FOR SNOW REMOVAL, HOWEVER A SNOW PLOW SWERVED TO AVERT A COLLISION WITH THE PA31.

    TF19 WAS HOLDING SHORT OF RUNWAY 25 AT MRI AND IN ERROR CALLED LHD FOR DEPARTURE. LHD LOCAL CONTROL (LC) CLEARED TF19 FOR TAKEOFF RUNWAY 32 WITHOUT DELAY. TF19 ACKNOWLEDGED THE INSTRUCTIONS AND TAXIED ONTO RUNWAY 25. LHD LC DID NOT OBSERVE TF19 ENTER THE RUNWAY AND INSTRUCTED TF19 TO CANCEL TAKEOFF CLEARANCE AND HOLD SHORT OF RUNWAY 32. TF19 ACKNOWLEDGED THE TAKEOFF CANCELLATION WHILE HOLDING IN POSITION ON RUNWAY 25 AT MRI. C152 WAS ON A HALF MILE FINAL TO RUNWAY 25 AND OBSERVED TF19 HOLDING ON THE RUNWAY. MRI TOWER INSTRUCTED C152 TO GO AROUND AS IT CROSSED THE THRESHOLD. C152 HAD ALREADY INITIATED A GO AROUND AND OFFSET TO THE LEFT SIDE OF THE RUNWAY. MRAI CONTROLLERS ESTIMATED AIRCRAFT WERE 25-50 FEET APART AT THE TIME OF THE GO AROUND. LHD LC FREQUENCY IS 126.8 AND MRI LC FREQUENCY IS 126.0. LHD LC REQUESTED VERIFICATION THAT THE TF19 WAS AT LHD. TF19 RESPONDED IN THE AFFIRMATIVE SO LHD CLEARED THE TF19 FOR TAKEOFF RUNWAY 32. MRI LC OBSERVED THE TF19 ENTER THE RUNWAY WITHOUT A CLEARANCE AND INSTRUCTED THE AIRCRAFT TO EXIT THE RUNWAY. THERE WAS NO RESPONSE. MRI LC OBSERVED THE TF19 SIT ON THE RUNWAY FOR 90 SECONDS THEN DEPART WITHOUT A CLEARANCE.

LOCAL CONTROL 2 ATTEMPTED TO CANCEL THE C172 TAKE-OFF CLEARANCE, BUT THE C172 WAS ALREADY AIRBORNE. GROUND CONTROL STOPPED THE C680 JUST SHORT OF TAXIWAY LIMA. C172 OVERFLEW C680 BY AN ESTIMATED 80 - 100 FEET. THIS EVENT OCCURRED AT HOT SPOT 1.

DH8D ENTERED RUNWAY 10R, STOPPING WITH THE NOSE 10 FEET PAST THE RUNWAY EDGE. A10 WAS DEPARTING RUNWAY 10R AND BECAME AIRBORNE PRIOR TO TAXIWAY ECHO. A10 PILOT REPORTED HE ROTATED EARLY, APPROXIMATELY 10 KTS BELOW NORMAL VR. A10 PILOT ESTIMATED HIS AIRCRAFT WAS AT AN ALTITUDE OF 25-30 FEET AS HE PASSES BY THE DH8D. A10 PILOT CONFIRMED THE DH8D WAS 10 FEET PAST THE RUNWAY EDGE LINE.

C182 WAS AT FULL THROTTLE ON TAKE-OFF ROLL WHEN THE OVERFLIGHT OCCURRED. CONTROLLER TRAINING IN PROGRESS.

CRJ7 PROCEEDED STRAIGHT AHEAD ON TAXIWAY KILO 12 AND ENTERED RUNWAY 4R WHILE B737 WAS DEPARTING. GC INSTRUCTED THE CRJ7 TO HURRY ACROSS THE RUNWAY FOR DEPARTING TRAFFIC. B737 WAS UNABLE TO ABORT TAKEOFF AND PASSED THROUGH THE EAST SIDE OF THE INTERSECTION AIRBORNE. CLOSEST PROXIMITY WAS 940 FEET LATERAL AND ABOUT 135 VERTICAL WHEN THE CRJ CLEARED THE RUNWAY EDGE AND THE B737 WAS HALFWAY BETWEEN TAXIWAYS VICTOR AND KILO 12.
