On 29 April 2022, a Piper PA-28-140 departed from Dryden Regional Airport, Ontario, intending to fly to Marathon Airport under visual flight rules (VFR). Following the flight's departure, the aircraft was reported overdue at 5:39 am UTC.

A search mission conducted by the Joint Rescue Coordination Centre (JRCC) later located the wreckage. The aircraft had struck terrain approximately 20 nautical miles southeast of Sioux Lookout, Ontario. The impact resulted in substantial damage to the aircraft, and all 4 occupants sustained fatal injuries.
