Occurrence
On 29 April 2022, a Piper PA-28-140 aircraft departed Dryden Regional Airport, Ontario, on a visual flight rules (VFR) flight to Marathon Airport, Ontario. The aircraft was reported overdue at 5:39 am UTC. A Joint Rescue Coordination Centre (JRCC) mission subsequently located the aircraft; it had collided with terrain approximately 20 nautical miles southeast of Sioux Lookout, Ontario (CYXL). The aircraft was substantially damaged.
Occupants and Injuries
The four occupants of the aircraft sustained fatal injuries.
Investigation
The Transportation Safety Board of Canada conducted a class 4 investigation into the occurrence. Class 4 investigations are limited in scope, and while the final report may contain limited analysis, it does not contain findings or recommendations. The investigation was generally completed within 220 days.
Damage
The aircraft was substantially damaged as a result of the collision with terrain.
