Introduction
On 28 June 2023, a Boeing 777-368ER aircraft, registered HZ-AK28, operated by Saudi Arabian Airlines as Flight SVA-806, was involved in a serious incident at VGHS Airport (Hazrat Shahjalal International Airport) in Dhaka, Bangladesh. The incident was classified as a runway excursion during landing on Runway 14. The Aircraft Accident Investigation Committee of Bangladesh (AAIC-BD) was notified by air traffic control and subsequently initiated an investigation.
Investigation
The Head of AAIC-BD formed a two-member Aircraft Accident Investigation Team (AAIT), designating himself as the Investigator-in-Charge (IIC) and the Member (Operations) as the associated member. The team proceeded to the site to secure evidence. The operator, Saudia Airlines, submitted a Mandatory Occurrence Report. The investigation was conducted in accordance with Section 19 of the CA Act 2017 and ICAO Annex 13. The AAIC-BD issued a preliminary report on 26 July 2023 and a draft final report on 15 March 2024, inviting comments from states and organizations. Boeing provided comments, which were incorporated. The final report was dispatched on 23 June 2024.
Findings and Cause
The final report does not include a stated probable cause for the incident. The investigation focused on identifying underlying causes rather than apportioning blame. Safety recommendations were issued to applicable states and organizations.
Conclusion
The AAIC-BD released the final report and safety recommendations with the sole objective of preventing future aircraft accidents and incidents.
