What happened
During the takeoff roll from runway 14 at Bandar Lampung-Branti Airport, the captain perceived an engine failure. In response to this perceived malfunction, the flight crew initiated an aborted takeoff procedure. While attempting to decelerate on the runway, the aircraft type was unable to come to a complete stop before reaching the end of the pavement.
The aircraft subsequently overran the runway and collided with a concrete wall. The impact caused the plane to crash into a small hill situated approximately 138 meters beyond the runway threshold. The accident resulted in 2 fatalities, including the pilot and one passenger, while 12 injuries were reported among the other occupants. A total of 46 individuals on board escaped the wreckage without injury. The aircraft was destroyed during the collision.
Findings
Investigations into the incident revealed that no engine failure actually occurred during the takeoff sequence. Instead, the accident was attributed to improper pre-takeoff configuration resulting from a failure by the crew to adhere to established checklists and inadequate flight preparation.
