Incident Overview
On August 30, 2021, at 17:33 UTC, an Embraer ERJ-190-100 IGW with registration YV2912 was preparing for departure from the International "Simón Bolívar" Airport (SVMI) in Maiquetía, Venezuela. The aircraft, operated by Consorcio Venezolano de Industrias Aeronáuticas y Servicios Aéreos S.A. (Conviasa), was en route to Cancún, Mexico, with 111 people on board, consisting of 7 crew members and 104 passengers.
Event Description
During the takeoff roll on runway 09/27, the flight deck displayed an "AT FAIL" (Autothrottle Failure) message along with an Automatic Flight Control System (AFCS) Flight Fail indication. At this point, the aircraft had reached a speed of 80 knots, and the Captain observed a loss of airspeed indication. The crew decided to abort the takeoff, executing standard procedures and returning to the apron under their own power. All occupants were unharmed, and the aircraft sustained no physical damage.
Investigation Findings
The investigation by the Joint Board for the Investigation of Civil Aviation Accidents and Incidents (JIAAC) focused on the Air Data System. Maintenance records indicated that the ADSP component was "On Condition," meaning it lacked a specific expiration date but required replacement upon failure. The board determined that the installed component had reached the end of its useful life, resulting in a transient fault that caused the momentary loss of data in the system's computational elements.
Following the incident, maintenance personnel performed troubleshooting according to the Fault Isolation Manual and replaced the NIC + PRO module and Smart Probe No. 1, both obtained via cannibalization from other aircraft due to inventory shortages. The investigation concluded that the failure was attributable to the end-of-life status of the ADSP component.
