What happened
On December 12, 2022, at 21:35 UTC, an Embraer ERJ 190/100 IGW, registration YV2944, was performing a scheduled flight from General Santiago Mariño International Airport in El Yaque to Simón Bolívar International Airport in Maiquetía. The aircraft, operated by CONVIASA, was carrying 103 passengers and 5 crew members.
During the final approach phase while executing an ILS procedure for runway 10L, the flight crew extended the nose landing gear. Upon doing so, the Engine Indicating and Crew Alerting System (EICAS) displayed an "LG LEVER DISAG" message, indicating that the nose gear was unsecure. The crew notified the tower of the nose landing gear (NLG) extension failure indication. Following the execution of necessary checklists, the crew performed a second approach and successfully landed the aircraft at SVMI. The aircraft vacated the runway under its own power, and all 108 occupants were reported uninjured, with no damage to the aircraft.
The investigation
The JIAAC investigation included interviews with the pilot in command and the first officer, as well as a review of crew licenses and medical certificates. Investigators also examined maintenance records and performed photographic and video documentation of corrective actions. Maintenance personnel conducted functional tests on proximity sensors 1 and 2, adjusting them according to the Aircraft Maintenance Manual (AMM).
