Incident Overview
A scheduled passenger flight from Bali to Surabaya resulted in a runway excursion following the activation of the thrust reverser after landing. The aircraft deviated to the right, ran off the runway, and sustained severe damage. All persons on board were evacuated successfully.
Pre-Flight Information
Before the flight departed from Bali, the crew was briefed that the left engine's thrust reverser system was not operational. The flight to Surabaya was conducted without any notable events.
Approach and Landing
The flight crew performed a normal approach to runway 10 and landed the aircraft according to established procedures. The landing was uneventful until after touchdown.
Thrust Reverser Activation and Excursion
After the aircraft landed, the crew engaged the thrust reverser. Immediately, the aircraft began to veer to the right. The crew took action to counteract the deviation, but the aircraft continued to move off the runway. It lost its undercarriage during the excursion and came to rest in a grassy area alongside the runway.
Aftermath
All 144 occupants evacuated the aircraft safely. Examination of the aircraft revealed damage beyond repair.
