On the morning of July 26, 2015, a privately owned Piper PA-64-350P, bearing registration JA4060, was involved in a fatal accident in Chōfu City. The aircraft had just departed from Runway 17 at Chōfu Airport when it struck a private residence located in Fujimi Town.

The impact caused the aircraft to be destroyed and ignited a fire. The blaze spread to the house hit by the plane and caused damage to nearby residential structures.
